If you click the exit beta button there is a form.

Exit beta takes me straight to the old version without any form or anything showing.

If you aren't getting the form, you can write [e-mail] with any thoughts or comments you have and they'll be forwarded to the dev team, who are gleefully swimming in mail as we speak.
