Discreet-ish. At least in my state the cashier had to pull out a book and make sure the constraints weren't violated. It's a slow process and when there's a line you can feel all the eyes on you.

How recently? With software the register should take care of that automatically, just like knowing what things need what level of sales tax charged.
It happened to the person I was shopping with in California in 2012 or perhaps 2011.
