I hate being in team meetings where we're discussing a failure and avoid talking about individual actions/responsibilities leading up to the issue. Don't make it personal, but do make clear what went wrong and who made mistakes. If everyone is responsible, no one is responsible. Principle #15 here weighs in on this:
