What is the best MCP server in the market for adding long-term memory to AI conversations? Bonus points if it's open source and bonus points if supported by third-party benchmarks.

Claude Code has a “#” prefix to send messages to its long term memory.

Beyond that, I just tell Claude to maintain its own memories in a collection of markdown files via CLAUDE.md. It also maintains an index.
