I have myself started practising this at the beginning of this year when I changed job. I have all my notes in git repo as well as a changelog file. But changelog is more like a timetrack file. In repo I have also commit.sh script [1] (for which I have also desktop shortcut) and at the end of work day I simply run it and (after a few more confirmations) turn of laptop and go home.
