Arguably, I think exercise should be #1. EDIT: Assuming they are ordered, otherwise ignore me. Cancer is about 20% of your chance of death. Intuitively exercise helps with many things on this list. The other (roughly) 25% is cardiovascular/cerebrovascular events.
