Why was there an open drain hole that can fit a person in the first place? Was there no grate?

Luckily for him there wasn't, a grate would have sliced him to bits at that pressure differential
Aren't there drains specifically meant to limit the possible pressure differential, after people's organs got removed by ones than didn't have that feature?

Relying on a human operator for safety when you could make something inherently safe still seems like a design issue.

I haven't been to many pools, and only ones with a shallow end, but I don't think I've ever just seen an open pipe hole
