Yeah, even at companies with a lot of red tape and procedure, in the end it's just a group of people and people make stupid oversights. Doesn't mean there shouldn't be public shaming, but usually stuff like this isn't some nefarious plot. Source: have worked at large institutions and seen stupid mistakes almost go through except for sheer blind luck |
