Short summary of other pieces of info:

- images are quite likely to dissappear, especially if there were many consecutive prompts;

- sometimes full chats dissappear;

- sometimes my prompt is replaced with a different prompt and I get totally random content;

You mean in mid-conversation or after you finish a conversation?

Either one. I first noticed in older conversations, but it happens also in current chats

I stopped using OpenAI a while ago because they never seemed good at running production.

Good at research, not so much for resilience & reliability
