I always have one of more pages of loose-leaf on my desk. It’s messy, but basically the lowest barrier to entry. And if I’m somewhere else and write something down, it goes in the pile with the other notes when I get back.

A few times a year, I open an org-mode file and start summarizing my notes. Those sheets then get put away, to help keep things a little less messy.
