Mandatory? Unless I'm seriously out of the loop, I don't think there are mandatory annual wellness checks in the US?

mandatory that you are offered one by your insurer, not mandatory that you show up to one

PPACA mandates it as a minimum essential coverage benefit, i.e. you don't pay extra for it. Included benefit as part of the plan.
