The most reliable number I can find for patients without comorbidity is ~.4/100k[0] (so about 1/250,000). A study specific to dental anesthesia produced a similar number (3 per 1,000,000)[1]. I couldn't find any mortality numbers for local anesthesia, but it would pretty much require an allergic reaction, and I know from past experience I don't have that allergy.
