You can still run your own DoH server, it's just an HTTPS server with a DNS backend.

But in that case, you will not benefit of DoH3, it's limited to Cloudflare and Google servers right now.

Ah I just noticed the footnote.

I'm still trying to add http3 to my DNS server and can't test DoH. But based on the other comments, it looks like a custom DNS server will use DoH if supported?
