Can anyone provide technical details about this? I'm curious about how a company as large as Microsoft manages signing certificates and how this failure happened.

It's probably because it's a large company, this kind of failure occurred.

I bet they'll find the scapegoat, while the whole chain of managers will keep their positions and/or get promoted.
