I wrote everything in markdown, then rendered to html with multimarkdown (https://fletcherpenney.net/multimarkdown/), and then converted to pdf with wkhtmltopdf (https://wkhtmltopdf.org/) and to epub/mobi with Calibre's ebook-convert (https://calibre-ebook.com/)
