There was only a very short time with hardware issues on USB. What really plagued it in the beginning was that Play and Play (the big selling point for USB 1.0) was really mostly Plug and Pray.

In tech support we called it Plug and Die

Thankfully it more or less sorted itself out, but I still secretly curse 'driver first' installations, so counterintuitive for users
