My work concerns safety in high-hazard industries, where lessons learned analysis (also called operational experience feedback) is a very important organizational process. It's also well-known to be difficult, not so much related to the technical tools used for recording/sharing incidents and lessons, but mainly in terms of organizational culture (learning, blame, psychological safety and speaking up).
