I remember reading a while back that when an app gets taken down, those decisions are made by people very higher up at Apple and not low level employees/reviewers. I really doubt Tim Cook doesn't know about such a huge deal.

Maybe for some category of apps. I can't believe that's the general case - there are too many to bother the higher-ups with.

The app was not taken down. It never made it through the initial review process to get to the store in the first place.
