There's no separate scroll wheel — it scrolls by turning the ball around the z axis, so it's purely optical. It works very well unless you need to keep the x,y absolutely fixed while scrolling. I can't comment on the wrist rest; wrist extension causes me trouble, so I have mine sunken into a customized keyboard tray to place the ball near keycap height.
