Sounds like a terrible idea if you ever plan to sell your product in another market. Regulators aren't typically big fans of trying to deliberately skirt their regulations.
Nice idea, but I would imagine there are a few App store terms and conditions that would not allow a medical app dressed as a game. Then again, if it's Steam based...
