What happened to allowing the industry to regulate itself?

The overwhelming majority of the industry is in favor of these regulations being formal. No one wants space debris.

What does that look like, exactly? Should SWARM have been "allowed" to launch? Who is the "allowing" body? What rules do they all follow?

I'd say this is a case where the industry didn't regulate itself.
