Have you ever sat yourself down and considered whether you aren't the problem?

If you know someone who you regard as a good manager, try and persuade them to come in for a couple of days (pay them for this, obviously) and do a sanity check on your management style.

I had management coaching years ago and the results were pretty hard to take - but it was an invaluable experience for me.
