There's a good comparison chart/spreadsheet of VPN providers at [1] - among other things, it can filter on anonymous sign up and payment. There are certainly VPNs available that you can sign up for over Tor, and pay for with Bitcoin. However, some bitcoin payment services block Tor IP addresses; tools for VPN-over-Tor can be clumsy; and some sites that accept Bitcoin process the payments manually so it can take a day or two.
