I'm in a similar position but if I leave the company will die with 100% certainty. My cofounder has no idea and the company is doing well and growing but it is a living nightmare. So many uncertainties and we're in a problematic market where there are a lot of room for errors with dire consequences.

I'll give up a lot of potential money if I leave but I don't care. The biggest problem is that my cofounder would never forgive me. I can't even imagine his reaction.

You should really talk to your cofounder... If your company relies so much on you, and if you're burnt out, then your company is NOT doing well at all. You should let your cofounder know, so that you can take action together before the whole thing blows up. Your cofounder will not forgive you for hiding the fact that the company is in a dire situation.
That sounds horrible. Are there any steps you can take to enable your cofounder, or someone else, to take over some of the highest risk tasks?
