to help clarify this -- they pay 1% more than the normal rate. to CAH's

The 101% of cost reimbursement is a different payment model than used for bigger hospitals (Prospective Payment Systems https://www.cms.gov/medicare/medicare-fee-for-service-paymen... ).

And no one else pays anywhere near 100% of 'reasonable costs' (they pay more, or in the case of Medicaid, less). So I wouldn't call it the normal rate.
