I highly recommend Zeke Emanuel's "Reinventing American Healthcare" which covers the current status of the US healthcare system and the many reasons about why it turned out the way that it did. It's fairly comprehensive for a primer but is very accessible. If I remember correctly, the book covers the question you're asking directly. I don't have the book handy otherwise I would summarize it for you. |
