Just came to post this. All the usual words come to mind, "cycle of abuse", "failing system", "slip through the cracks", "individual choice".

If we keep saying the words and the problem still keeps happening, we are saying and doing the wrong things.

Ideas welcome. Good ones only.

such statements do not encourage creative feedback ("bad" idea for one is the genius idea for another, and even a bad "bad" idea may help generate a great one)
